Membership Application Forms and Membership Renewal Forms for 2012-2014 are now available. 

If you are not a Member then you must complete a Registration Form.

If you are already Registered then you will need to complete a Membership Renewal Form by March 31^st 2012 and send it to DSISO (details in the Form)

MEDICAL INFORMATION UPDATE

You are required to notify DSISO if there are any changes in your medical information or AAI status on the Membership Renewal Form

If any change requires medication then a Medical Letter is required, otherwise details can be given in a letter from a Parent/Carer.

The Membership Fee is $50 or €40 for the period 2012 - 2014.

This will be collected at the next DSISO World Championships (Loano Italy) from the Team Manager from your Country

DO NOT send it to DSISO since this will incur Bank Charges to you and DSISO

NOTE:  You can still renew in Loano even if you will not be there personally.  Just make sure you give it to the Team Manager

NOTE:   Failure to Renew your Membership will result in you being removed from the Membership Database and you will not be allowed to enter any DSISO events or submit any Records until you have sent in a Membership Renewal Form.
